Easy to set up. It even has a multiple choice option where the customer can select more than one field. We only need one form at the moment, so free is a great price. Awful experience. No customer service / support whatsoever. I started using this for my Shopify site - worked well, but I needed a small tweak, to be able to have a form offer 40 choices rather than the standard 20.
